Blockade Runner sells them.
As does Dixie Gunworks, and theirs includes the worm.
From my experience, avoid the Sgt tool. The worm threads are too small to fit either my Armi or Euroarms ramrods. (Although if anyone can tell me how the mainspring vice works, I'd love to know!) The Pvt tool fits fine. And the cone wrench works on a Sharps, without pulling the block, if you grind the tip to about a 45* angle.
